The Golden Rule of AI Documentation
If AI could have written it by reading only the code, it should not exist as a comment. Format: Status, Context, Decision, Consequences. One page max. Use AI to draft the template, human to fill in the reasoning.

Make documentation that updates itself. Executable specs, auto-generated API docs from code annotations, integration tests that double as usage examples. When the code changes, the docs change automatically because they ARE the code.
